IT Equipment Checkout Policy
Use of Equipment
- Technology equipment is intended to be used for official business only. Equipment for personal use is not provided.
- Equipment may be checked out to FJV departments for official FJV business purposes.
- Items reserved and not picked up before the reservation time are subject to being released to another user.
- In most circumstances, the maximum check out period is five (5) successive days with the weekend counting as one (1) day. Failure to return equipment on time may result in loss of borrowing privileges or other consequences.
- Lost, stolen and damaged equipment will be charged to the borrowing department at current replacement cost.
Department Check-out Procedure
- Staff may request reservations for equipment by submitting a Happyfox service desk ticket only. Please give us at least a 3 business day notice for presentations and trainings, weekends are not included. If there are multiple events and times, please submit a separate service ticket for each instance.
- Only FJV staff are entitled to checkout privileges, unless otherwise approved by the leadership team.
Department Check-in Procedure
- The staff member which checked out the equipment must return the equipment to the Information Technology department.
- Information Technology department will check the equipment and make sure that the equipment is in the same condition in which it was initial checked out to staff.
Late Return of Equipment
- Two late returns are allowed per quarter. If a user is late a third time during the quarter, their checkout privileges are revoked until the start of the next quarter.
- This policy applies to all users in the organization.
Damaged, Lost or Stolen Equipment
- Any damaged or missing equipment/accessories will be charged to the staff members department. (Parts and Accessories Replacement Costs.)
- Lost or stolen equipment will be charged to the staff members department at the current replacement cost.
- Equipment must be returned in the same physical and electronic condition in which it was received.
Father Joe’s Villages is not responsible for damage to user's personal equipment used in conjunction with FJV equipment.
